Job Summary
Eligibility Specialist coordinates admissions information and intake, provides information and referrals to community agencies, supports enrollment with eligibility, promotes the Senior CommUnity Care Program to potential participants and community agencies, gathers documentation for Medicaid approval, ensures compliance with State Medicaid rules and regulations (Medicaid, LTC, Spousal Impoverishment and Trusts, Food Stamps, OAP), guides participants through financial eligibility processes, and assists with applications for certification and recertification; role involves home, nursing homes, hospitals, or other settings as part of assessing potential participants and facilitating care enrollment.
Required Qualifications
- Bachelor's degree in a health and human services field or equivalent years of experience related to public benefits
- Two years of experience in marketing, with more emphasis on sales, and/or admissions role for a health care provider, or in an information and referral resource role within the senior networks
- Minimum one year of experience working with the frail or elderly population
- Valid driver's license and means of transportation within 100 miles daily
