Job Summary
Eligibility Specialist / Case Manager to provide comprehensive care coordination for adults with intellectual/developmental disabilities and Autism Spectrum Disorder enrolled in Medicaid Waiver. Develops Person-Centered Service Plans, coordinates and monitors services, facilitates ISP implementation, and ensures alignment of Behavioral and Crisis Plans with participant needs. Responsibilities include completing assessments, coordinating with care entities, researching alternative solutions, maintaining confidentiality, attending required trainings and recertifications, and regular collaboration with supervisors to tailor supports. Travel to participants is expected up to 50% of duties.
Required Qualifications
- BS/BA degree in related course work
- Experience with Microsoft Office
- Must maintain proficiency in company sponsored training and certifications
- Maintain CPR/First Aid certification and updated state related Clearances
- Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration
