Job Summary
Eligibility Specialist / Case Manager in Goose Creek, SC provides coordination of services and development of person-centered care plans for adults with developmental and physical disabilities, including ISP development, service brokerage, monitoring of outcomes, and collaboration with care coordinators to arrange housing, medical, social, vocational, and community supports. The role requires assessing participant needs, creating and updating care plans, ensuring informed provider selection, coordinating with behavioral and crisis plans, reporting incidents, ensuring confidentiality, and traveling as needed (approximately 50%). Must have a BS/BA, proficiency with Microsoft Office, CPR/First Aid certification, valid driver’s license with appropriate insurance, and ongoing training/certifications as required by program regulations.
