Eldercare Nurse Clinic Manager

Review and revise department policies, manage data collection for clinical and financial performance, and monitor daily staffing levels. Oversee the Eldercare PACE Model of Care, including direct supervision of Center C.N.A.'s, clinical oversight, and competency evaluations. Coordinate home health and long-term care services, manage clinic flow, triage, and appointment bookings, and ensure compliance with federal and state regulations. Conduct comprehensive nursing assessments, supervise medication administration, and communicate with weekend and after-hour staff on the on-call rotation. Assist in program development, strategic planning, and monthly leadership meetings to review key performance indicators and operational metrics.
