Job Summary
Facilitate high-quality life care planning services by coordinating facility placements, coaching families on advocacy, and assessing clients' functional levels to develop care plans. Partner with attorneys and long-term care planners to ensure the quality of elder law services while maintaining confidentiality and resolving client concerns with urgency. Research community resources, attend Interdisciplinary Team Meetings to document decisions, and represent the firm to healthcare facilities through visits and relationship-building. Collaborate with marketing staff to promote services via seminars and events, ensuring the client's voice remains central throughout the journey.
Required Qualifications
- Bachelor's degree in Social Work or a related field (or equivalent work experience)
- 2–3 years of experience in a professional environment with strong administrative, coordination, or client-facing responsibilities
- Strong organizational skills and ability to manage multiple priorities with consistent follow-through
- Exceptional verbal and written communication skills
- High professionalism, integrity, and commitment to confidentiality
- Proficiency with Microsoft Office Suite and standard office technology
