Educational Evaluator

Conduct brief academic and school history reviews with patients and families, then formulate written reports detailing learning strengths, weaknesses, and specific recommendations for educational management. Consult with interdisciplinary team members to contribute to treatment plans, recommend appropriate educational placements, and collaborate to secure resources for Individualized Education Programs or 504 Plans. Act as a liaison between the treatment team and Local Educational Agencies by participating in school meetings and providing clear discharge recommendations. Confers with parents regarding findings and remediation strategies, while referring patients to Hospital Instruction and providing in-service instruction to schools and community agencies. Participates in regular interdisciplinary team activities, research, and outpatient consultations to ensure smooth client transitions and programmatic follow-through.
