ED RN Care Coordinator

Job Summary
Conduct psychosocial and functional status assessments for patients, initiating chronic disease management reviews and identifying discharge barriers. Collaborate with physicians, interdisciplinary teams, and families to develop individualized discharge plans, manage length of stay, and facilitate post-acute referrals. Proactively identify delays in diagnostic or treatment plans, resolve obstacles to discharge, and ensure consensus with payers and care teams. Document all clinical and psychosocial interactions in the electronic medical record, track avoidable days, and participate in interdisciplinary rounds. Requires RN license with one year acute care experience; supports department goals and precepts students as assigned.
Required Qualifications
- Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
- RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
- BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
- Minimum 1 year nursing experience in the acute care setting
- Excellent written and verbal communication skill
- Must possess maturity, self-confidence, objectivity, and positive attitude
- Self-directed with the ability to function well under stress, handle change, and function in a fast-paced environment
- Strong assessment, interview, organizational and problem-solving skills
- Knowledge regarding local, state and federal regulations
- Knowledge of community and state-wide resources and programs
- Ability to work collaboratively with physicians, members of the care team, and the patient/family to assist with progression of care through their transition to the next level of care
- All certifications are required upon hire unless otherwise stated
