ED RN Care Coordinator

Job Summary
Assess transitional care needs, coordinate care across the continuum, and engage with patients and families to assure care needs are met. Initiate assessments of chronic disease management, psychosocial risk factors, and resource availability upon discharge. Collaborate with physicians, the interdisciplinary care team, and utilization review to establish discharge plans, manage length of stay, and facilitate post-acute referrals. Identify barriers to timely disposition, resolve delays proactively, and document all clinical and psychosocial interactions in the electronic medical record. Participate in interdisciplinary rounds to confirm estimated discharge dates and recommend best levels of care transition. Requires RN license, one year acute care experience, and BLS certification. Day shift in the United States.
Required Qualifications
- Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
- RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
- BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
- Minimum 1 year nursing experience in the acute care setting
- Excellent written and verbal communication skill
- Must possess maturity, self-confidence, objectivity, and positive attitude
- Self-directed with the ability to function well under stress, handle change, and function in a fast-paced environment
- Strong assessment, interview, organizational and problem-solving skills
- Knowledge regarding local, state and federal regulations
- Knowledge of community and state-wide resources and programs
- Ability to work collaboratively with physicians, members of the care team, and the patient/family to assist with progression of care through their transition to the next level of care
- All certifications are required upon hire unless otherwise stated
