Job Summary
Conduct comprehensive psychosocial assessments, crisis intervention, and high-risk screening for emergency department patients. Facilitate patient/family care conferences, manage post-acute placements, and educate families on community resources including mental health services and hospice. Maintain up-to-date resource lists, collaborate with interdisciplinary teams, and uphold ethical standards while adhering to federal and state regulations. Ensure continuity of care through safe discharge planning and advocacy for patients and families navigating complex healthcare needs.
Required Qualifications
- 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting
- LMSW license from the AZ Board of Behavioral Health Examiners (MSW: AZ)
- Master's Degree from a School of Social work accredited by the Council of Social Work Education
- Ability to work independently as needed with a high degree of detail orientation
- Ability to work efficiently in a fast-paced environment with changing priorities
- Ability to work collaboratively with an interdisciplinary care team
Desired Qualifications
- Minimum 3-Year Post-MSW healthcare experience
