Job Summary
ECM Registered Nurse role overseeing CalAIM Enhanced Care Management program operations. Provides clinical leadership and supervision to LVNs and Lead Care Managers, guides care coordination workflows, ensures compliant, high-quality service delivery, leads MDT meetings, collaborates with internal/external stakeholders, mentors staff, develops training and educational materials, and ensures accurate documentation in eCW. Required Qualifications
- Valid CA Registered Nurse license
- Bachelor of Science degree preferred
- 3-5 years' experience working as a Registered Nurse
- Working knowledge of health information systems and data needed to monitor/assess utilization, performance management, and health outcomes
- Experience preparing and delivering presentations
- Experience managing a team preferred
- Extensive knowledge of development of evaluation tools, and evaluation of health programs, data analysis, and report writing
- Strong knowledge of community health evaluation tools, approaches, frameworks, and methods
- Bilingual English/Spanish preferred (read, write, speak)
