Job Summary
Coordinate medical and non-medical supportive services for members experiencing homelessness, including conducting Health Needs Assessments, developing Care Plans, and engaging clients in treatment. Manage a caseload of 17-25 individuals by facilitating medication reviews, scheduling appointments, providing transportation, and advocating with healthcare professionals and hospital staff. Collaborate with behavioral health providers, LTSS specialists, and community supports to ensure comprehensive care while using motivational interviewing and trauma-informed practices. Maintain accurate documentation on clients, assist with data collection systems, and report progress to the multidisciplinary team. Work within the Mercy House team to implement core values and develop resident programs.
Required Qualifications
- Bachelor's Degree in Social Work (or related field)
- Valid CA driver's license
- Proof of insurance
- Reliable vehicle
- Knowledge of how to use Microsoft programs
- Computer literacy
- Ability to lift items weighing up to 50 pounds

Desired Qualifications
- Experience working in chemical health, mental health, and co-occurring disorders
- Preferred minimum of 1 year of experience in care coordination and case management for the following populations: Individuals/Families experiencing homelessness, Individuals at risk for avoidable hospital stays (High Utilizers), Individuals with Serious Mental Health and/or SUD, Adult Nursing Facility Residents Transitioning to the Community, Adults with intellectual or developmental disabilities
- Experience with HMIS Data Entry/CalOptima Health
- Experience with Housing First and Harm-Reduction strategies
- Experience with Motivational Interviewing
- Experience with Patient-Centered and Trauma-Informed Care
- Experience with Crisis Intervention & Prevention strategies
- Experience in engaging and assisting individuals/families with acute mental illness & substance use to access/navigate health care as well as other needed services
- Fluency in Spanish
