Job Summary
The Enhanced Care Management (ECM) Lead Care Manager coordinates both medical and non-medical supportive services for clients at risk of homelessness. Responsibilities include managing a caseload, conducting Health Needs Assessments, and creating individualized Care Plans. The role involves outreach, enrollment of members, advocating for clients with healthcare providers, and coordinating care among various providers. Experience in case management, outreach, and familiarity with trauma-informed care and motivational interviewing is preferred. A Bachelor’s Degree in Social Work or related field is required, along with a valid CA driver’s license.
Required Qualifications
- Bachelor’s Degree in Social Work or related field
- Experience working in recovery, mental health, and co-occurring disorders
- A minimum of 1 year of experience in care coordination or case management
- Experience with individuals or families experiencing homelessness
- Experience with high utilizers of hospital or emergency services
- Experience with individuals with serious mental illness and/or substance use disorders
- Valid CA driver’s license and proof of insurance
- Comfortable utilizing own vehicle for client visits
- General dependability and effective communication skills
