Job Summary
Supervise Lead Care Managers and oversee day-to-day operations of the Enhanced Care Management and Community Supports programs to ensure performance and compliance. Monitor caseload distribution, capacity, and workflow while managing referral, enrollment, and disenrollment processes across contracted Managed Care Plans. Coordinate with plan representatives on requirements and reporting, conduct quality assurance reviews of documentation, and support audit preparation. Track key performance indicators, prepare reports, and lead onboarding and training for the care management team. This role requires full-time Monday through Friday presence at the Moorpark corporate office with regular travel to the San Fernando location.
Required Qualifications
- Minimum 2 years of experience in Enhanced Care Management, Medi-Cal Managed Care, or a related care coordination program
- Familiarity with CalAIM and Managed Care Plan ECM/CS Provider Manuals
- Must have a California Driver's License, satisfactory driving record and a vehicle that is insured in accordance with state/or organization requirements
- Must be willing and able to commute between both office locations
- Must be available for regular travel 1-2 days per week to the San Fernando office
- Must be available for on-site presence to support daily supervision, team collaboration, in-person training, and program oversight activities
- Strong organizational, communication, documentation, and leadership skills
- Ability to manage multiple priorities and meet Managed Care Plan deadlines
Desired Qualifications
- Prior supervisory or team lead experience
- Bachelor's or Master's degree in one of the following: Social Work, Psychology, Counseling, Rehabilitation, Gerontology, Sociology, or Health or Human Services
- Fluent in Spanish and English
