ECM Clinical Manager (LCSW/LMFT)

Job Summary
ECM Clinical Manager oversees remote clinical oversight of Enhanced Care Management staff delivering telehealth-based care coordination. Responsibilities include supervising a team of ~10-15 direct reports, overseeing client outreach, enrollments, and Individual Care Plans; serving as escalation point; driving KPIs; collaborating with Director(s) to implement changes; providing clinical oversight (approximately 30%), including sign-offs on care plans and leading IDT meetings; providing direct care management as needed; and traveling up to 20-25% for trainings and meetings. Must have an active LCSW or LMFT license in California and be willing to obtain additional licensure as needed. Work schedule is Monday–Friday 8:30am–5:00pm PT with flexibility for evenings/weekends; remote role with preference for candidates in specified states (AZ, CA, CO, FL, GA, IL, NV, NC, OR, TX, UT, WA) and Pacific Time.
Required Qualifications
- Active, unrestricted LCSW or LMFT license in California
- Willingness to obtain additional state licensure as needed
- 2 years of people management experience
- 2-3 years of care management experience
- 1-2 years of post-acute experience
- Proven leadership and change-management capabilities
- Strong organizational and communication skills
- Proficiency with Microsoft and Google applications; Salesforce a plus
- Working knowledge of lean Six Sigma preferred
