ECM Case Manager

ECM Case Manager position in the Illumination Health + Home network is responsible for assessing clients enrolled in Permanent Supportive Housing (PSH) programs, linking them to resources, developing individualized service plans, and monitoring progress toward goals. Responsibilities include trauma-informed, client-centered care; collaboration with care team members and healthcare providers; connecting clients to essential services (medical, mental health, transportation, food assistance, behavioral health resources); coordinating discharge planning; developing and reassessing comprehensive service plans; conducting home visits and wellness checks; supporting employment and benefits activities; advocating for housing, healthcare, education, and emotional wellbeing; maintaining HIPAA-compliant documentation in HMIS/KIPU; and ensuring housing habitability standards. The role requires strong confidentiality, professionalism, and cultural sensitivity, with expectations to use a personal vehicle for client visits. Preferred qualifications include bilingual Spanish skills and experience with HMIS/KIPU, Housing First models, and Microsoft Office.
