Job Summary
ECM Care Manager, RN coordinates clinical care for high-risk patients across medical, behavioral, and social domains. Responsibilities include reviewing and planning patient care via EMR, developing and evaluating individualized plans with providers, collaborating with physicians and care-team members, and mentoring Care Management Team staff. The role requires utilization-management knowledge, patient activation support, and the ability to apply evidence-based practices, with duties spanning coordination of resources, policy/procedure development, and ensuring appropriate level of care. Schedule is Monday–Friday, 7:30 am–4:00 pm at Indio Clinic, Indio, CA.
Required Qualifications
- Education: Associate of Science Degree in Nursing required. A Bachelor in Nursing (BSN) degree is preferred.
- Licensure/Certification: Current and unrestricted California Registered Nursing License, Current CPR/BLS certification (American Heart Association or Red Cross accredited program). EPIC certification for the module hired into. Valid California driver's license, and auto insurance.
- Experience: 3+ years of acute care, hospital-based experience required.
- Interpersonal Skills: Proficiency in MS Office Suite; English proficiency; ability to communicate effectively; teamwork and leadership capabilities.
