Job Summary
Family Nurse Practitioner providing comprehensive primary care and geriatric services in the Eau Claire, WI area. Conduct patient assessments, physical exams, and follow-up visits; diagnose and manage acute/chronic conditions (diabetes, hypertension, asthma); perform admission history and physicals; complete discharge/medication reconciliation; provide preventive care and patient education; collaborate with nursing staff and interdisciplinary teams; support behavioral health needs with whole-person care; travel between facilities may be required; in-person work with potential hybrid arrangement.
Required Qualifications
- Master’s or Doctorate degree in Nursing
- Current Family Nurse Practitioner certification
- Active Wisconsin Nurse Practitioner license
- Strong communication and clinical decision-making skills
- Experience in primary care, geriatrics, or long-term care preferred, but motivated new providers are encouraged to apply
