Job Summary
Coordinate and monitor service delivery for children with developmental delays, serving as the primary point of contact to assist families in accessing needed services. Facilitate IFSP/FSP development, conduct follow-ups to ensure service alignment, and make referrals while maintaining accurate documentation for Medicaid and IDEA compliance. Educate families on their rights, promote caregiver competence, and support transition planning across service systems. Participate in multidisciplinary team meetings and collaborate with providers to identify goals. Requires a bachelor's degree in social work or related field, one year of experience with infants and toddlers, and current CPR/First Aid certification. Flexible schedule with travel required.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
- Current CPR/First Aid certification
