Job Summary
Serve as primary point of contact for families, assisting them in accessing needed services and coordinating service delivery. Conduct follow-ups to ensure services are provided and monitor alignment with Individualized Family Service Plans. Facilitate evaluations, develop IFSP/FSP documents, and participate in multidisciplinary team meetings to help families identify goals. Educate families on rights, promote caregiver competence, and support services in natural environments. Maintain accurate records, complete service notes, and ensure compliance with Medicaid and IDEA Part C. Work with families, providers, and agencies to coordinate across service systems and support transition planning. Community-based work with flexible schedule and travel required.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
