Job Summary
Coordinate and monitor service delivery for children with developmental delays, serving as the primary point of contact for families to access needed services. Facilitate IFSP/FSP development, conduct evaluations, and make referrals to ensure services align with family goals and promote caregiver competence. Educate families on their rights, support transition planning, and maintain accurate documentation while ensuring compliance with Medicaid and IDEA Part C. Work with multidisciplinary teams and community-based providers in a flexible schedule with travel required.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
- Current CPR/First Aid certification
