Job Summary
Coordinate and monitor service delivery for children (birth to age 6) with developmental delays, serving as the primary point of contact to assist families in accessing needed services. Facilitate IFSP/FSP development, conduct follow-ups to ensure service alignment, and make referrals to schedule appointments with providers. Educate families on their rights and safeguards while promoting caregiver competence through multidisciplinary team meetings and goal identification. Maintain accurate documentation to ensure compliance with Medicaid and IDEA Part C. Work in a community-based setting with flexible scheduling and travel required.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
- Current CPR/First Aid certification
