Job Summary
Coordinate evaluations and assessments for children with developmental delays, serving as the primary point of contact to assist families in accessing needed services. Facilitate Individualized Family Service Plan development, conduct follow-ups to ensure service delivery, and make referrals to schedule appointments. Educate families on their rights, promote caregiver empowerment, and support services in natural environments while maintaining accurate records and ensuring compliance with IDEA Part C and Medicaid. Participate in multidisciplinary team meetings, monitor service alignment, and coordinate across service systems. Community-based work with flexible schedule and travel required.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
- Current CPR/First Aid certification
