Job Summary
Coordinate evaluations and assessments while serving as the primary point of contact for families with children (birth to age 6) experiencing developmental delays or disabilities. Facilitate IFSP/FSP development, make referrals to schedule appointments, and conduct follow-ups to ensure services align with family goals. Educate caregivers on rights and safeguards, promote empowerment, and support service delivery in natural environments through multidisciplinary team collaboration. Maintain accurate documentation ensuring compliance with Medicaid and IDEA Part C standards. Community-based work requires travel and involves flexible scheduling.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
- Current CPR/First Aid certification
