Job Summary

Coordinate and monitor service delivery for infants and toddlers with developmental delays, serving as the primary point of contact to assist families in accessing needed services. Facilitate Individualized Family Service Plans (IFSP) and multidisciplinary team meetings while conducting follow-ups to ensure services align with family goals. Educate caregivers on their rights and safeguards, promote empowerment, and support service transitions across community-based systems. Maintain accurate documentation ensuring compliance with Medicaid and IDEA Part C regulations. This role requires a Bachelor's degree in Social Work, Special Education, or related fields, plus one year of experience working with children with disabilities. Travel is required for community-based work, and the position offers a flexible schedule.

Required Qualifications

  • Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
  • Minimum 1 year of experience working with infants, toddlers, or children with disabilities

Desired Qualifications

  • Experience with Early Intervention, IDEA Part C, or pediatric services
  • Knowledge of IFSP/FSP development and child developmental milestones
  • Experience working with families and community-based services
  • Current CPR/First Aid certification
