Job Summary
Coordinate evaluations and assessments for children with developmental delays, then facilitate IFSP/FSP development and reviews. Serve as the primary point of contact for families to assist with accessing services, make referrals, and schedule appointments. Conduct follow-ups to ensure service delivery aligns with plans and monitor services across systems. Educate families on rights, promote caregiver competence, and support services in natural environments. Maintain accurate records and ensure compliance with Medicaid and IDEA Part C. Participate in multidisciplinary team meetings to help identify goals and support transition planning.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
