Job Summary
Coordinate and monitor service delivery for children with developmental delays, serving as the primary point of contact for families to access needed services. Facilitate IFSP/FSP development, conduct evaluations, and make referrals while ensuring services align with family goals and legal compliance under IDEA Part C and Medicaid. Educate families on their rights, promote caregiver competence, and support service delivery in natural environments through multidisciplinary team collaboration. Maintain accurate documentation and oversee transition planning. Community-based work with flexible schedule and travel required.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
