Job Summary
Coordinate and monitor service delivery for children with developmental delays or disabilities and their families. Serve as the primary point of contact to assist families in accessing needed services, making referrals, and scheduling appointments. Conduct follow-ups to ensure services are provided and maintain accurate records while ensuring compliance with Medicaid and IDEA Part C. Facilitate IFSP/FSP development, participate in multidisciplinary team meetings, and support transition planning. Educate families on rights and promote caregiver competence in natural environments. This role requires a Bachelor's degree in Social Work, Psychology, or related fields, plus one year of experience working with infants and toddlers. Travel is required for community-based work, and a flexible schedule is available.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
- Current CPR/First Aid certification
