Job Summary

Coordinate evaluations and assessments for children with developmental delays, serving as the primary point of contact for families to access needed services. Facilitate IFSP/FSP development and reviews while making referrals, scheduling appointments, and conducting follow-ups to ensure service delivery aligns with plans. Educate families on rights and safeguards, promote caregiver competence, and support services in natural environments through multidisciplinary team meetings and transition planning. Maintain accurate records and ensure compliance with Medicaid and IDEA Part C. Community-based work with flexible schedule and travel required.

Required Qualifications

  • Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
  • Minimum 1 year of experience working with infants, toddlers, or children with disabilities

Desired Qualifications

  • Experience with Early Intervention, IDEA Part C, or pediatric services
  • Knowledge of IFSP/FSP development and child developmental milestones
  • Experience working with families and community-based services
  • Current CPR/First Aid certification
