Job Summary

Coordinate and monitor service delivery for children with developmental delays or disabilities, serving as the primary point of contact for families. Assist families in accessing needed services, make referrals, and schedule appointments while conducting follow-ups to ensure services are provided. Facilitate IFSP/FSP development, participate in multidisciplinary team meetings, and educate families on their rights and safeguards. Maintain accurate records and ensure compliance with Medicaid and IDEA Part C. Work with families, providers, and agencies to support transition planning and promote caregiver competence in natural environments.

Required Qualifications

  • Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
  • Minimum 1 year of experience working with infants, toddlers, or children with disabilities

Desired Qualifications

  • Experience with Early Intervention, IDEA Part C, or pediatric services
  • Knowledge of IFSP/FSP development and child developmental milestones
  • Experience working with families and community-based services
  • Current CPR/First Aid certification
