Job Summary
Coordinate and monitor service delivery for children with developmental delays or disabilities and their families. Serve as the primary point of contact to assist families in accessing needed services, conduct follow-ups to ensure services are provided, and make referrals for appointments. Facilitate IFSP/FSP development and reviews while participating in multidisciplinary team meetings to help identify goals and outcomes. Educate families on rights and safeguards, promote caregiver competence, and support services in natural environments. Maintain accurate records, complete service notes, and ensure compliance with Medicaid and IDEA Part C. Work with families, providers, and agencies to coordinate across service systems and support transition planning.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
- Current CPR/First Aid certification
