Job Summary
Coordinate and monitor service delivery for children with developmental delays or disabilities, serving as the primary point of contact for families to access needed services. Facilitate IFSP/FSP development, conduct follow-ups to ensure service provision, and make referrals to schedule appointments. Educate families on their rights, promote caregiver competence, and support services in natural environments while maintaining accurate documentation and ensuring compliance with Medicaid and IDEA Part C. Work with multidisciplinary teams to plan goals and support transition planning. Community-based work with flexible schedule and travel required.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
- Current CPR/First Aid certification
