Job Summary
Coordinate and monitor early intervention services for children birth to age six with developmental delays, serving as the primary point of contact for families. Facilitate Individualized Family Service Plans (IFSP/FSP) development, conduct evaluations, and make referrals to ensure aligned service delivery. Educate families on their rights, promote caregiver competence, and support transitions across service systems while maintaining accurate documentation for Medicaid and IDEA Part C compliance. Work with multidisciplinary teams, providers, and agencies in a community-based setting with flexible scheduling and travel requirements.
Required Qualifications
- Bachelor's degree in one of the following fields or related area: Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or Health-related field, Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities
Desired Qualifications
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
- Current CPR/First Aid certification
