Job Summary
The Early Intervention Services (EIS) Specialist identifies individuals living with HIV/HCV and links them to medical care through DAP Health clinics and community referrals. Responsibilities include delivering individualized HIV counseling and HIV prevention education (including harm reduction, condoms, lubrication, and ART), completing assessments to identify social determinants of health barriers, facilitating referrals to Prevention Navigator for PEP/PrEP, removing barriers to care, coordinating referrals to medical services, maintaining accurate documentation and EHR (HCC, Epic), meeting clients on a walk-in basis, providing post-appointment support (lab coordination and same-day prescription pickup), and connecting clients with relevant internal/external resources. On-site at a DAP Health location in Oceanside, CA, with ability to travel within southern California; requires valid driver’s license and insurance; ability to lift 24 pounds; office-based/hybrid travel as needed.
Required Qualifications
- Bachelor's degree OR experience in medical or health-related fields; social work, counseling, nursing, public health, or other health education
- Current American Heart Association or American Red Cross accredited BLS certification
