Job Summary
Service Coordinator for Early Intervention who travels to conduct home visits for families with children from birth to 3 years, assisting with IFSP development and access to services, coordinating eligibility determinations and screenings, maintaining client records, and collaborating with internal and external partners to promote coordinated, family-centered care. Responsibilities include data entry, reporting, participating in committees/trainings, promoting program services, and ensuring compliance with program rules and confidentiality. Requires strong communication, organizational skills, and ability to work across community resources in Muskingum County; valid driver’s license and reliable transportation are expected.
Required Qualifications
- Associates Degree in Nursing, social work, special or early childhood education, child development or related field as specified by the Help Me Grow or Early Intervention program requirements
- Two (2) years of experience working with infants / toddlers and their families and performing service coordination or equivalent with experience working with diverse populations in a community based setting preferred
- Valid driver’s license, good driving record, and access to reliable transportation
- Basic computer knowledge with competency in Word, Excel, Google products and other job related software
