Job Summary
Schedule Individual Family Services Plan (IFSP) meetings, create individualized family service plans, and document progress toward established goals for children with developmental disabilities. Coordinate support by finding early intervention providers, assisting families in obtaining necessary services, and advocating through mediation with community resources like Denver Public Schools. Maintain accurate documentation of plans, progress, and child-specific information in Salesforce/Unicorn and Astro systems while ensuring compliance with Department of Human Services regulations. Drive to home visits and perform follow-ups on department assignments. Requires a valid driver's license, personal vehicle, and proof of motor vehicle insurance.
Required Qualifications
- Bachelor's degree in Early Childhood Education/related field
- Minimum of five years related experience
- Ability to read, write and speak English fluently
- Excellent verbal and written communication skills
- Excellent organizational skills
- Valid driver's license
- Proof of motor vehicle insurance
- Personal vehicle in good operating condition for use during work
- No major violations in the past three years
- No more than two moving violations in the past three years
- Ability to meet and maintain agency driving requirements and operate agency vehicles
Desired Qualifications
- BILINGUAL - Spanish / English
- Ability to read, write and speak Spanish fluently
- Knowledge of resources, policies, and history of EI systems at local, state, and national levels
- Experience working with children ages zero to three
- Two years case management experience working with people with developmental disabilities and their families
