Early Intervention Home Base/ Social Worker

Job Summary
Early Intervention Social Worker provides home and community-based individual and collateral therapy sessions, caregiver coaching, and family training sessions for Birth-3 clients, conducts assessments, develops individualized plans, coordinates services, advocates for clients, and collaborates with an interdisciplinary team. Requires NYS certification/licensure and experience with Birth-3 population; strong communication, organizational skills, and ability to work within NYC EIP policies.
Required Qualifications
- NYS Certification or License/Registration in discipline with at least 2 years of experience providing therapeutic services to the Birth-3 population
- DOH Approval Letter- DOH Approval requires 1500 hours of service with children under 5
- Statewide Central Register Clearance
- Justice Center Screening
- Liability Insurance
- Knowledge of NYC EIP Policies & Procedures
