Job Summary
Act as a liaison between educators, schools, families, and clinicians to manage case files for students with IEPs or IFSPs. Accept, disseminate, and track evaluations while managing initial, reassessment, and supplemental evaluations from start to submission. Coordinate 6-month IFSP reviews, annual reviews, and enrollment protocols alongside the EI Director and SPED Coordinator. Send, track, and receive prescriptions timely and ensure all required documentation remains current for Special Education students. Complete systematic tasks in IEP Direct, Cleartrack, iCentral, and EI Hub. Manage telephone communications for service referrals and conduct home visits and school district meetings within the tri-county area.
Required Qualifications
- Valid New York State Driver's License
- Must have excellent writing skills
- Must be an excellent communicator
- Must have a working knowledge of computer programs including but not limited to Word, Excel, PowerPoint
- Authorized to work in the United States
- Travel for home visits and school districts in the tri-county area is required
- Must be able to lift children
- Must be able to lift, push, or pull in excess of 30 pounds
Desired Qualifications
- Bachelor's degree in a health or human services field preferred
