Early Childhood Case Manager

Deliver direct care services, care coordination, caregiver coaching, and Social Determinants of Health (SDOH) linkage for families of children 0–6 enrolled in CCBHC early childhood services. Work in conjunction with the Early Childhood Therapist to implement the plan of care, reinforce skills in natural settings, and support rapid access and crisis follow-up. Responsibilities include providing quality, strengths-based case management for SED children and families; coordinating with child-serving agencies; rapid engagement and barrier solving (transportation, childcare); coordinating with therapists to implement interventions; in-home caregiver coaching; SDOH screening/linkage (housing/food/transportation/benefits); documentation and assessments aligned to timelines; afterhours/weekend on-call rotation; and other duties as assigned. Job qualifications include a Bachelor’s degree in relevant fields or equivalent combination with one year of experience substituted for one year of education; preferred experience with Kansas early childhood systems, Head Start/IEP/IFSP supports; ability to work with children and families in mental health and human services settings; background checks, alcohol/drug screening, and motor vehicle record checks; and ability to work in a flexible schedule. Typical physical demands include sitting, occasional bending, exposure to communicable diseases, and ability to work under high stress.
