E&M Coder - PHYS

Review, analyze, and code medical record documentation for medical, diagnostic, and procedural information to assign correct ICD-10, CPT, and HCPCS codes with maximum specificity. Abstract demographic and coding data into the information system, review documentation for medical necessity, and audit orders and claims before submission to minimize denials. Prepare reports and provide technical guidance to physicians and departmental staff regarding coding issues and errors. Primary responsibility covers all Evaluation and Management (E/M) leveling and ancillary services performed in an office setting. One or more certifications such as RHIA, RHIT, CCA, CPC, or CCS are required.
