Job Summary
Conduct thorough evaluations of new patients, including collecting pertinent medical history and performing specific physical assessments to create tailored treatment plans. Educate patients and their families about evaluation findings and collaboratively set achievable goals that align with their individual health needs and circumstances. Implement targeted treatment protocols based on evaluation results and ongoing assessments, adapting to the evolving needs of patients in real-time. Document patient care activities accurately and in a timely manner, including initial evaluations, treatment goals, daily progress notes, and discharge summaries. Engage with a multidisciplinary team of healthcare providers, ensuring smooth communication regarding patient progress and making necessary adjustments to treatment as conditions change. Participate actively in community outreach and marketing initiatives to promote the center's services and enhance patient access to care.
Required Qualifications
- Doctorate degree in Physical Therapy (DPT) from an accredited institution
- A valid Physical Therapy license in the state of Florida
- BLS certification through the American Heart Association
- Strong interpersonal skills to effectively communicate with patients, families, and colleagues
- Commitment to providing high-quality standards of care and maintaining patient confidentiality
Desired Qualifications
- A minimum of two years of direct experience in a physical therapy setting, preferably in outpatient or home care
- Experience with electronic health records (EHR) systems for documentation and patient tracking
- Prior involvement in clinical education and mentoring of Physical Therapy students or new staff
