Job Summary
Coordinate crisis intervention and stabilization for individuals presenting to the Department of Social Services with Opioid Use or Polysubstance Abuse Disorders. Maintain current Care Plans, verify compliance with Cattaraugus County and Intandem documentation requirements, and establish verbal, face-to-face, electronic, or written contact with clients within their natural environment at regulatory frequencies. Assist clients in transitioning from inpatient or residential settings to community-based care through service linkages, while monitoring treatment progress and assessing personal goals.
Required Qualifications
- Bachelor's Degree in a Human Services field
- Associate's Degree
- Two (2) years' experience working in direct care human services position
- Functional knowledge of community resources
- Education and Interest in training in Substance Abuse
- Openness to learning new skills and technology
- Valid New York State Driver's License that meets Agency standards
- Reliable Transportation
- Strong organizational and analytical skills
- Ability to think creatively and be results oriented
- Ability and willingness to abide by agency policies and procedures
- Ability to deal with multiple tasks efficiently
- Ability to represent self and agency in a professional manner
- Ability to engage and build a productive rapport with the people we serve and with outside providers/agencies
