Job Summary
Provide person-centered care by building meaningful relationships, assisting with daily living activities like personal care and meals, and supporting community involvement and independent living skills. Advocate for residents' rights and monitor their health and well-being while reporting changes. Complete required documentation, participate in training and team meetings, and maintain a valid driver's license for transportation. Work evening shifts in a residential group home setting supporting up to four people with identified needs.
Required Qualifications
- Ability to read and write in English
- A valid driver's license
- Safe driving record
- Ability to pass a Motor Vehicle Check
- Ability to pass a DHS background check
- 40% FTE or greater
- Remain up to date on all trainings
- Maintain employment in good standing
- Work all scheduled shifts
