Job Summary
Implement personalized support and foster independence for individuals with developmental disabilities by executing daily living activities, including personal hygiene, meal preparation, and medication administration. Monitor and document comprehensive plans of care, goals, and objectives while maintaining accurate progress notes, incident reports, and medication logs. Facilitate community outings, recreational activities, and appointments to promote inclusion and meaningful interactions. Utilize positive behavior support strategies to address challenging behaviors and collaborate with interdisciplinary teams, including healthcare professionals and family members, to ensure holistic support. Advocate for the rights and preferences of persons supported while adhering to OPWDD, NYSJC, and organizational policies as a mandated reporter.
Required Qualifications
- High school diploma or equivalent
- Valid NY/NJ/CT/PA Driver's License with 6 months of experience and 6 or less points
- High School / G.E.D.
Desired Qualifications
- experience in direct care or human services
- CPR and First Aid certification
- additional training in developmental disabilities, behavior management, or healthcare
- Strong interpersonal and communication skills

- Commitment to providing compassionate care and promoting dignity and respect for individuals with developmental disabilities
