Job Summary
Provide direct care and training to consumers in community residences, implementing behavior management plans and ensuring personal hygiene needs are met. Administer medications, transport residents, and maintain accountability for all consumers within the assigned setting. Perform extensive documentation including daily logs, progress notes, activity schedules, and monthly inventories. Conduct unit-wide cleaning, cooking, and laundry supervision while participating in interdisciplinary meetings and monthly fire drills. Work the third shift every other weekend in the Pee Dee C'field or Cheraw areas. Requires valid driver's license, First Aid/CPR certification, and ability to lift with assistance. Buys groceries and supplies weekly; prepares food and service provision logs.
Required Qualifications
- High School Diploma or equivalent
- Acquire and maintain certification in First Aid, CPR, and CPI (Non-Violent Crisis Intervention)
- Subject to annual TB test
- Valid Driver's license from state of residence
- Ability to operate motor vehicle for 15 passengers or less
- Must be available for 3rd shift every other weekend
- Must be available in Pee Dee C'field or Cheraw Areas

- Must be able to maintain accountability for all consumers within the assigned setting
- Must be able to demonstrate knowledge of consumers' actions during that time frame
- Must be able to ensure consumers' needs are met through the implementation of goals, behavior management, personal hygiene, oral hygiene, social adjustment, leisure, and special activities
- Must be able to ensure consumers' well-being
- Must be able to report inappropriate or maladaptive behaviors to residents as needed
- Must be able to implement unit wide and individual behavior management plans consistently
- Must be able to perform all DSP paperwork
- Must be able to write up new sheets for the month
- Must be able to maintain cleanliness in the unit through supervision of the chores, house cleaning, cooking and laundry as scheduled or needed
- Must be able to participate as a member of the interdisciplinary team and other consumer-related meetings as needed
- Must be able to provide information and assist with evaluation
- Must be able to administer medications to residents within CHESCO Services medication administration guidelines
- Must be able to make needed copies for direct care paperwork
- Must be able to attend in-service training and staff meetings as required
- Must be able to transport consumers as needed
- Must be able to conduct monthly fire drills on each shift
- Must be able to conduct disaster drills as required
- Must be able to provide coverage to other shifts and community programs as needed
- Must be able to buy groceries and supplies weekly or as needed
