Job Summary
Provide ongoing overnight and morning support for self-care routines, including toileting, feeding, showering, dressing, and diapering. Ensure health, safety, and well-being by assisting with household chores, implementing treatment plans, and administering medications per AMAP policy. Transport supported individuals to day programs or job locations using safe driving practices. Document all activities, incidents, and progress via Medisked or paper logs. Wake and greet individuals with a positive tone, assist with meal preparation, and participate in inter-disciplinary team meetings. Mentor new Direct Support Staff and adhere to Federal and New York State regulations.
Required Qualifications
- Qualified New York State Driver's license that is also qualified in accordance with agency's insurance standards
- Successful completion and maintenance of AMAP status
- Ability to communicate and understand written and oral directives/information
- excellent team player with a positive attitude
- Qualified driver status
