Job Summary
Implement and monitor comprehensive functional assessments, life plans, and behavior support plans for individuals with developmental disabilities. Provide direct assistance with daily living activities, including personal hygiene, meal preparation, medication administration, and transportation. Facilitate community outings, recreational activities, and social events to promote inclusion while accompanying persons to appointments and healthcare follow-ups. Maintain accurate records, daily progress notes, and incident reports to track achievements and ensure compliance with OPWDD, NYSJC, and organizational policies. Collaborate with interdisciplinary teams, including therapists and family members, to develop holistic care plans. Advocate for the rights and preferences of supported individuals, promoting self-advocacy and empowerment in decision-making processes.
Required Qualifications
- High school diploma or equivalent
- Valid NY/NJ/CT/PA Driver's License with 6 months of experience and 6 or less points
- High School / G.E.D.
Desired Qualifications
- experience in direct care or human services
- CPR and First Aid certification
- additional training in developmental disabilities, behavior management, or healthcare
- Strong interpersonal and communication skills

- Commitment to providing compassionate care and promoting dignity and respect for individuals with developmental disabilities

- Must be a custodian and mandated reporter in accordance with OPWDD, NYSJC, and Organizational Policies
