Job Summary
Implement and monitor comprehensive functional assessments, life plans, and behavior support plans for individuals with developmental disabilities. Provide direct care for daily living activities including personal hygiene, meal preparation, medication administration, and transportation. Facilitate community outings, recreational activities, and social events while accompanying individuals to appointments and healthcare follow-ups. Maintain accurate records, daily progress notes, and incident reports to track achievements and ensure regulatory compliance. Collaborate with interdisciplinary teams, healthcare professionals, and family members to develop holistic care plans. Advocate for the rights and preferences of supported individuals while serving as a mandated reporter in accordance with OPWDD and organizational policies.
Required Qualifications
- High school diploma or equivalent
- Valid NY/NJ/CT/PA Driver's License with 6 months of experience and 6 or less points
- High School / G.E.D.
Desired Qualifications
- experience in direct care or human services
- CPR and First Aid certification
- additional training in developmental disabilities, behavior management, or healthcare
