Job Summary
Implement personalized support and foster independence for individuals with developmental disabilities through daily living activities, medication administration, and structured skill-building. Monitor and document comprehensive plans of care, including functional assessments and behavior support strategies, while maintaining accurate incident reports and progress notes. Facilitate community outings, recreational activities, and healthcare appointments to promote inclusion and meaningful interactions. Collaborate with interdisciplinary teams and family members to develop holistic care plans, advocating for the rights and preferences of persons supported. Ensure compliance with OPWDD, NYSJC, and organizational policies as a mandated reporter, while responding flexibly to changing needs in a safe, nurturing environment.
Required Qualifications
- High school diploma or equivalent
- Valid NY/NJ/CT/PA Driver's License with 6 months of experience and 6 or less points
- High School / G.E.D.
Desired Qualifications
- experience in direct care or human services
- CPR and First Aid certification
- additional training in developmental disabilities, behavior management, or healthcare
- Strong interpersonal and communication skills
