Job Summary
Implement and maintain treatment plans for persons supported with developmental disabilities in medical and behavioral settings, focusing on person-centered approaches. Assist with activities of daily living, including personal hygiene, food preparation, and mobility support for individuals with physical limitations or medical conditions. Execute behavioral support and monitoring plans while managing complex medical needs such as wound care, feeding, and respiratory issues. Maintain accurate service records of daily activities, medication administration, and behavior data to ensure regulatory compliance with OPWDD, Justice Center, and COA standards. Facilitate community inclusion through participation in social events and recreational opportunities, utilizing authorized plans to de-escalate challenging behaviors and respond to crises. Transport persons to institutions, hospitals, and home visits, and attend Life Plan Meetings to review habilitation plans. Requires NADSP certification obtained within the probationary period, a valid driver's license, and experience working with adults with developmental disabilities.
Required Qualifications
- High School Diploma
- Valid NY, PA, CT and NJ Driver's License with 6 or less points
- NADSP Certification
- Knowledge of Developmental Disabilities
- Experience working with adults that have developmental disabilities
- Familiarity with regulatory requirements, custodians and standards governing residential programs for persons with developmental disabilities
- Capacity to organize time effectively, prioritize assignments, and maintain authentic/ contemporaneous records
- The ability to handle confidentiality within HIPAA effectively and tactfully with family/advocates, visitors, government agencies, and the public
- Ability to work effectively as part of a team, collaborating with coworkers to deliver continuity of care and dignity to those served
- Demonstrated strong commitment to safety
