Job Summary
Implement and monitor plans of care, including functional assessments and behavior support plans, for individuals with developmental disabilities. Provide direct assistance with daily living activities such as personal hygiene, meal preparation, medication administration, and transportation. Facilitate community outings, recreational activities, and appointments to promote independence and social inclusion. Maintain accurate records, progress notes, and incident reports while collaborating with interdisciplinary teams and advocating for individual rights and preferences. Administer medications according to prescribed schedules and monitor for adverse reactions.
Required Qualifications
- High school diploma or equivalent
- Valid NY/NJ/CT/PA Driver's License with 6 months of experience and 6 or less points
- High School / G.E.D.
Desired Qualifications
- experience in direct care or human services
- CPR and First Aid certification
- additional training in developmental disabilities, behavior management, or healthcare
- Strong interpersonal and communication skills

- Commitment to providing compassionate care and promoting dignity and respect for individuals with developmental disabilities
